Main duties of the job About the Role In this pivotal role, you will: Lead and support a team of Child Health Hub Coordinators, ensuring smooth coordination of services. Act as the main point of contact for GPs, social care, therapists, nurses, and other MDT professionals, facilitating information-sharing and effective teamwork. Ensure that referral pathways are clear and efficiently managed to provide the best possible patient care. Support service improvement by gathering and analysing data, helping to enhance patient engagement and co-production. Play a crucial part in developing transformational leadership within the team, ensuring continuous professional growth and high-quality service delivery. About us Probationary Period This post will be subject to a probationary period.
