Work within a multidisciplinary framework, undertaking a key-working role for non-complex patients as triaged by registered staff. Liaise closely with medical, nursing, allied health professionals, social care, acute and voluntary sector staff to promote greater understanding of the service. Support Staff in the delivery of therapeutic programmes on an individual basis or as a group. This will involve work from a variety of different disciplines with the post holder needing to prioritise and negotiate to ensure a manageable caseload is achieved in a timely manner. Implement, evaluate, and modify intervention, as directed. Be responsible for a delegated non-complex caseload following assessment by the registered staff, carrying out interventions, including goal setting, outcomes, risk assessment and discharge. However for complex patients follow the treatment plan and feedback on progress to registered staff and highlight need for re-assessment when needed. Monitor progress, taking account of the environment and adjust own clinical interventions accordingly Undertake risk assessments as part of everyday clinical practice, complete relevant documentation and seek guidance as necessary.
