An exciting opportunity has become available to provide nursing care to patients within the community environment and plan the workload for junior staff and support the team leader.
Main duties of the job
1. To provide nursing care to patients within the community environment and plan the workload for junior staff and support the team leader.
2. To participate in the delivery of nursing care and support to clients within the community setting and have the necessary confidence and competence to work autonomously.
3. To work under the direction of the Integrated Team leader and to be responsible for assessment and management of the health needs of an identified population including end of life care and patients with long term conditions and/or complex nursing needs.
4. To support and wherever possible maintain patients in their own preferred place of care, promoting self-care enabling them to maximise their independence and optimise their quality of life 7 days per week throughout the year.
5. Maintain close liaison with all agencies and professions involved in client care and support, involving the client and his family in all discussions and decision making.
6. To act as Key worker and coordinate care for patients requiring Long Term Condition management or End of Life Care at home.
7. Plan duty rosters, ensuring that appropriate level of cover is available to maintain client safety, and carry out the required nursing and care procedures.
Minimum Requirements
This vacancy is open to those who have a current NMC PIN to practice as a Registered Nurse or Midwife in the UK, or those who are currently enrolled on a course which means they expect to receive their full NMC PIN within the next 3 months. Please note an NMC PRN PIN is not a full NMC PIN and would not fulfil the essential criteria for this role.
The NCA can no longer accept applications from International Nurses and Midwives who are yet to complete all competencies needed to receive a full NMC PIN to practice in the UK as we have paused our OSCE programme indefinitely.

* Up to date professional portfolio demonstrating evidence of reflective practice
* Ability to travel to different geographical locations across the Trust footprint in line with service needs
* Full UK driving license with willingness to use own car for work purposes
Knowledge, Skills, Training and Experience
* Demonstrate excellent communication skills both written and verbal
* Ability to plan and organise own work
* Aptitude to work cohesively within a team
* Computer literacy skills Awareness of community working and care pathways
* V150 prescribing.
* Good interpersonal skills Leadership potential
* Management of change Relevant Community experience
