Job summary We have an exciting opportunity within the All Age Disability service for a Family Support Worker, one full time post, 37.5 hours per week, working 5 out of 7 days with some unsociable hours. This roles involve working directly with children and young people who have complex disabilities such as Autism and Severe Learning Disabilities and who at times present with challenging behaviours. You will support and enable these young people to access community-based activities, work alongside their families within the family home providing advice and support in all areas of family life. Provide specialist support in relation to the child's disability. Work closely with all professionals involved with the family including the Social Worker, Health, and Education Professionals. You will be expected to attend all meetings in relation to the young people you support, write reports to support the Social Work assessments, including contributing to parenting assessments. At times our young people do present with challenging behaviours, full training is provided to support you with this side of the role. Having a good and proven understanding of behaviour and working in a person-centred way is essential. Main duties of the job To enable children and young people with complex and severe disabilities to access short breaks within their community. To offer advice and support to families of children with complex and severe disabilities and work alongside all partner agencies. To develop, monitor, implement and review the Service Provision Agreement in line with the child/young person's Care Plan. To provide a flexible and person-centred service to the young person and their Carers. To develop and review Risk assessments for young people on your case load. To assist the Social Worker in the completion of Parenting Assessments, Child in Need Visits, agency referrals. To deliver training to partner agencies and families in relation to understanding disabilities. To provide early help intervention and support for families through the Early Bird Course. To respond to urgent support requests from Social Workers where intervention is focused on maintaining the child/young person remaining at home.
