Main duties of the job
1. Assess and provide high standards of initial assessment input to patients in the community.
2. Liaise with MDT members in treatment and management care plans and work closely with therapists.
3. Support OT/PT with recommendations for a patient-centered approach to achieve rehabilitation goals.
4. Create and evaluate all care plans implemented; possess a sound knowledge in discharge planning and ability to make further referrals into the community as required.

Job responsibilities

Person Specification
Qualifications
* Diploma/Degree in Physiotherapy/Occupational Therapy.
* HCPC Registration.
Travel
* Must be able to drive and own a car for work purposes with business insurance.
Experience
* 2-3 years UK NHS experience.
