Main duties of the job
1. Assess and provide high standards of initial assessment input to patients in the community.
2. Liaise with MDT members in treatment and management care plans and work closely.
3. Support OT/PT with the recommendations for a patient-centered approach to achieve their rehab goals.
4. Create and evaluate all care plans implemented, and should have a sound knowledge in discharge planning and the ability to make further referrals into the community as required.

Shift Patterns: 37.5 hours, Mon - Fri, 8.00 am - 4.00 pm
