Working within designated clinical areas, the post holder will facilitate patient discharge providing a high-quality service in line with Trust standards. They will be able to communicate sensitively and effectively with a wide range of people including patients, families, colleagues, and agencies external to the Trust.
The post holder will manage patient flows within a defined area and will be responsible for prioritising work, reviewing working practices to ensure timely and efficient patient access to the Trust's services, delivery of the patient administration pathway from the decision to admit the patient and throughout the inpatient process.
Main duties of the job
Manage and co-ordinate discharge & patient flows within Chapel Allerton CSU and promote timely decision making to adhere to safe patient placement.
To facilitate timely and appropriate patient discharge from Chapel Allerton CSU wards to home, alternative external health or social care provision, or transfer to the appropriate in-patient clinical service.

Job responsibilities
Take a lead role in the creation of capacity to meet demand and liaise with Discharge Lead and Matrons when insufficient demand is created.
Participate in systems and processes that maintain and improve patient flows.
Develop effective partnerships with clinical and clerical staff and an environment which fosters open communication and participation.
Participate in developing effective partnerships with other CSUs and their Admission/Bed Management Teams that enables timely and appropriate patient placement.
Contribute to the development of operational policies/clinical protocols that optimise patient flows on all sites.
Facilitate referrals to primary care, social services, or third sector organisations.
Contribute to providing a service that maximises the use of community resources and facilitates the timely and appropriate assessment and/or admission of patients, for the GPs in Leeds.
Maintain accurate records in accordance with the Trust guidelines.
Report accidents, untoward incidents, and complaints.
Undertake duties on other wards or departments as and when required by service demands or patient need.
Act in such a way that safeguards the health and wellbeing of children and vulnerable adults at all times. Is familiar with and adheres to the LTHT safeguarding policies.
Work closely with clinical teams and external agencies to support effective patient discharge.
Promptly alerts the Discharge Lead of any delays or difficulties within a patient's discharge plan.
Promptly alerts Nurse in charge to difficulties in patient flow, placement, or when capacity issues arise within their responsible ward areas.
Collect and maintain accurate and up-to-date information regarding patient flows (acute and elective admissions, discharges, transfers, delays).
Ensure all patient and carer enquiries are dealt with in a friendly and professional manner.
Participate in daily MDT meetings to ensure seamless services are delivered.
Raise concerns with regards to risk, danger, malpractice, or wrongdoing by following the LTHT Whistleblowing policy and supporting processes.
Promote a positive learning environment where lessons are learnt from patient/carer complaints.
Actively participate in the development of own personal development plan and appraisal.
Complete mandatory and priority training within the correct time periods.
Person Specification
Experience
* Hospital & Health Care experience.
* Experience with patient flow, discharge planning, or process management.
* Able to communicate with staff at all levels in the organisation.
* Knowledge of Patient Discharge Pathways, Recovery Care Plan / Home First Programme / Making Every Day Count.
* Managing patient flows and discharges in a hospital setting.
Skills and Behaviours
