Chorley Central Primary Care Network are looking for an experienced HCA to join the PCN Care Home team.
You will be part of an established team delivering the Enhanced Care in Care Home (EHCH) service for the 16 Care Homes in the PCN
Main duties of the job
Working under the direct supervision of the PCN Nurse andstrictly in accordance with PCN EHCH Guidelines and protocols.
Your duties will include
Conducting 6 monthly care plan reviews.
Ensuring all residents have a Personalised Careand Support Plan (PCSP)
Administer Influenza and COVID vaccinationsunder appropriate PSD (vaccination training will be provided)
Working within and supporting amultidisciplinary team (MDT) approach
Phlebotomy is desirable but not essential (trainingcan be provided)
Hours are 9am -2pm Tuesday - Friday

Qualifications
Essential
7. NVQ Level 2 in Health and Social Care or equivalent
8. Ability to assess and manage patient risk effectively and safely
9. Time management and ability to prioritise workload
10. Able to analyse data and information, drawing out implications for the individual patient/impact on care plan
11. Able to establish and maintain effective communication pathways within the organisation.
12. Excellent interpersonal, verbal and written communication skill
13. Car driver/access to car
Desirable
14. Experience of using a medical software package
15. Qualified in Phlebotomy
Knowledge
Essential
16. Understanding of systems to gain an understanding of the health needs of the population as they relate to primary care
17. Understanding of evidence based practice
18. Knowledge of national standards that inform practice ( National Service Frameworks, NICE guidelines etc)
Desirable
19. Understanding and knowledge of policy developments related to the delivery of primary care services including General Practice, the GMS/PMS contract, Clinical Governance, Quality & Outcomes Framework
