The Role
This rewarding and exciting role will be key in ensuring that effective, efficient and appropriate care is planned and managed well for people and that this care achieves a positive impact to the recovery of the individual. You will need to work across Cheshire West and Chester, your responsibilities will include:
• Delivering excellent drug and alcohol street-based interventions to individuals who are rough sleeping, or at risk of homelessness and those in supported accommodation, to facilitate positive outcomes.
• Carrying a small case load of around 30 + clients – (though this will change on needs and by demand’’).
• Coordinating and delivering assessments and evidence-based interventions (brief and structured), recovery planning and aftercare planning.
• Working collaboratively with colleagues and key stakeholders, partners, across multidisciplinary teams, ensuring adherence with all Via policies and Safeguarding principles.
• Conducting assertive outreach across Cheshire West & Chester to engage people with links to the local Homeless Person’s Unit. This includes outreach with homelessness organisations and other partner agencies.
• Experience in working within substance misuse and homelessness.
• Providing a service that is tailormade to meet the needs of the individual and to understand that our clients’ needs an adaptable and responsive approach.
We are looking for someone who is passionate about what we do, and with a positive and engaging approach to people using our service.
An empathetic and non-judgmental approach is imperative, as this role involves working with a wide range of challenging issues and behaviours. Good time management and team working skills are essential in this fast-paced service.
