Acute Learning Disability Liaison Practitioner

This post has been designed to support the continued development of care practices and pathways to meet the needs of patients with learning disabilities and Autism (LD/A) in an acute setting.
Main duties of the job
The successful post holder will lead the delivery of the Trust LD and Autism strategy, working collaboratively across acute and community services, ensuring high standards of care for patients with Learning Disabilities.
* The Acute Learning Disability Liaison Practitioner is the lead subject matter expert for the Trust and will work closely with the Safeguarding Specialist Nurse Lead - Adults (And Learning Disability).
The post holder will be responsible for providing guidance to hospital colleagues on meeting the needs of people with learning disabilities and development of appropriate services within Kingston Hospital linking with health and social providers.
The post holder is the lead for developing care pathways, setting, monitoring, and evaluating standards of care in partnership with the ward or department staff to improve the quality of care received by individuals with a learning disability in the acute hospital setting.
The post holder takes the lead in supporting the provision of assessment, preparation, planning and co-ordination at points of attendance, admission and discharge. The post holder will assist with auditing the effectiveness of patient pathways in collaboration with all services.
The post holder will gain feedback from patients with learning disabilities and their carers.

Person Specification
Education and Qualifications
* Registered Nurse/Allied Health Professional Learning Disabilities
* Masters Level qualification of relevant subject
Experience
* Advanced knowledge of the needs of clients with multiple disabilities
* Significant post registration experience and working knowledge of up to date legislation in respect of learning disability services and acute services
* In-depth knowledge and understanding of the Mental Capacity Act, Best Interests and advocacy
* Experience of interagency working
* Able to identify service improvement needs and devise and implement corresponding actions
* Experience of significant contribution to innovative practice, change management and quality improvement
Skills and Abilities
* Ability to work to deadlines
* Ability to communicate effectively at all levels and across agencies
* Ability to write in-depth reports and present complex information in an understandable format
* Excellent interpersonal skills
