Interim Adults Social Worker for Triaging Referrals

Working Hours: 37 hours Monday – Friday, normal working hours

The team provides cover until 7pm on weekdays and 9am – 5pm on weekends, so some out of hours work is expected. Flexible working and the opportunity to work from home on a regular basis are also available.
Your role would be to triage referrals, providing urgent responses, offering advice and information, and signposting. The role involves triaging requests for social care assessments and safeguarding referrals. As a Senior Social Worker, you would also have the chance to attend multi-agency meetings and be a lead in championing new initiatives and team developments. It is expected that you will be a practicing Best Interest Assessor, AMHP, Practice Educator, or ASYE Assessor.
Most of the work is done over the phone, so you will need excellent communication skills. The work is very varied, with no two days being the same, and suits someone who enjoys a fast-paced work environment and has excellent time management and prioritization skills.
Key Responsibilities:
* Undertake comprehensive needs assessments.
* Plan, implement, and review care plans within the framework of the Care Programme Approach.
* Provide social work interventions to individual service users and carers as identified within the care plan.
* Contribute to the effective assessment and management of risk and protection of others. Liaise with the Probation Service and make referrals to the relevant MAPPA when appropriate.
* Undertake care coordination and social supervision of service users living in the Wakefield locality.
* Contribute to the provision of specialist multidisciplinary treatment programmes.
* Work with local and regional mental health services to meet service users' needs and facilitate effective transfer of care and discharge.
Key Requirements:
* DipSW or equivalent recognized by CCETSW.
* PQ1 (or a willingness to undertake within 12 months)
* GSCC Registration.
* Comprehensive understanding of types of mental disorder and effects of mental health problems on individuals and their carers.
* Aware of responsibilities under the Care Act 2014 in relation to the principles of the Act, prevention, and strengths-based conversations to determine eligibility and support planning with service users, families, and advocates.
* Aware of the principles of the Mental Capacity Act 2005 with experience of undertaking Capacity and Best Interest Assessments.
* 2 years post-qualifying experience.
* Experience of working within a fast-paced environment.
* Good risk assessment skills.
* A proven ability to manage a caseload effectively.
