This role will lead our Planned Care Team who are a bespoke multidisciplinary team delivering routine planned, follow up and recovery care for patients. The Planned Care team will be the largest team in healthcare made up of; a variety of registered and non-registered professionals ensuring our patients receive integrated and co-ordinated care. Your team and you will continue the care of men within the prison, taking over from the Early Days in Custody team after the first 10 days of a patients journey. Developing innovative ways to encourage this large patient group to manage their conditions and support their recovery. Work with the Head of Healthcare and Deputy Head of Healthcare to develop processes and methods to treat, inform and guide the patient group through their treatment, recovery and long term goals. Your team will be made up of dedicated; public health, mental health, primary care and substance misuse practitioners.
