Community Respiratory Staff Nurse - Easington/North Service

CDDFT Community Respiratory Nursing Service is pleased to advertise a permanent Community Respiratory Staff Nurse post working across within the North Durham and Easington. The successful postholder will deliver respiratory care to respiratory patients including COPD, Emphysema, Asthma, Bronchiectasis, and Interstitial Lung Disease, in out-patient clinics as well as in their own homes, supporting the specialist respiratory nursing service.
The successful candidate will utilise their outstanding organisation and communication skills to achieve best patient outcomes whilst being well supported by expert respiratory specialists to develop a range of competencies in delivering excellent respiratory care and support for this group of complex patients.
This post offers flexible working Monday - Friday 08:30-17:30.
Main duties of the job
* To support the respiratory specialist nursing team to manage complex respiratory patients in the community.
* Support community teams with complex respiratory patients.
* Take responsibility for the assessment of the health needs of patients under community respiratory care and manage a caseload of respiratory patients.
* Offer chronic disease management of respiratory patients including COPD exacerbation pathway patients and virtual ward.
* Signpost patients and colleagues to appropriate services and escalate as needed.
* Provide specialist advice to patients/carers.
* Maintain records.
* Accept responsibility for decisions made and actions taken.
* Skilled at assessing and managing risk.

MAIN DUTIES AND RESPONSIBILITIES
Management and Leadership
* Contribute to creating improvement, transformation, and innovation within the service in order to promote best practice.

Clinical Responsibilities
* Co-manage a case load of complex respiratory patients, under direction of the community respiratory nursing team, across the community respiratory specialist service, within clinic and patients' home environments, Monday-Friday 9am-5pm.
* Undertake holistic patient respiratory assessment and review including social and psychological well-being and progress treatment interventions and adapt interventions, within set protocols, to meet the changing needs of the patient.
* Develop, implement, and evaluate patient treatment plans.
* Communicate patient treatment plan changes to respiratory specialist nursing team.
* Understand basic respiratory anatomy and pathophysiology and the function and management of all respiratory conditions including mental well-being.
* Perform capillary blood gas testing and oxygen reviews.
* Provide long term condition care following the COPD pathway.
* Accurately record clinical data and understand its significance including heart rate and rhythm, respiratory rate, pulse oximetry, peak flow rate, and blood pressure.
* Complete effective record keeping which demonstrates the decision-making process, care planned and given, as well as patient outcomes in accordance with trust policy and legal requirements.
* Promote health and healthy informed choices.
* Review smoking status, provide very brief advice and refer to appropriate smoking service.
* Review and train inhaler technique to patients, carers, and other health care professionals.
* Participate in service improvement and development initiatives.
* Have awareness, knowledge and implementation of safeguarding processes in relation to children, vulnerable patients and carers.
* Understand the importance of research, audit, and quality improvement and participate in the monitoring of standards of care, and audit of nursing/therapy practice which is research-based and supports the clinical governance framework.
* Be responsible for the safe physical environment including equipment.
* Participate in and contribute to all activities associated with the teams response to current trends and changes within the NHS.
* Adhere to trust clinical governance and equality and diversity policy through ethical decision-making.
* Support and promote effective implementation of all Organisational Policies and Procedures.
* Work collaboratively with the existing respiratory specialist teams.
* Possess good IT skills and use of systems.

Statutory Registration
* Nursing Midwifery Council part 1 and 2 Registration.
Experience
* Knowledge of national and regional policies influencing respiratory disease.
* Post registration experience of working with respiratory patients.
* Experience of working in a community setting.
Qualifications
* Registered Nurse.
* Professional knowledge acquired through degree or equivalent level of demonstrable professional knowledge/experience.
* Accredited respiratory course, or willingness to work towards.
