Job summary Sheringham Medical Centre is looking to recruit a highly skilled advanced clinical practitioner (ACP) to join our innovative and patient focused team on a full-time basis. As an ACP, you will play a key role in providing expert clinical care, assessing, and prioritising patient needs, and managing caseloads, including treatment plans, referrals, and discharges. You will work closely with our multidisciplinary team to ensure the highest standards of patient care. Main duties of the job Manage patient care from initial assessment and diagnosis to treatment and follow-up. Provide care for nursing and residential homes, with home visits as required. Deliver safe clinical decision-making and expert care in collaboration with our team. Be the first point of contact within the practice for patients with undiagnosed problems. Provide home visits to patients as required. Provide weekly scheduled visits to care homes. Prescribe safe and appropriate medications. Prioritise health problems and intervene appropriately including initiation of effective emergency care. This role is suitable for paramedics, advanced nurse practitioners, or newly qualified nurse practitioners. Our practice serves a patient list of 9,400 and provides a full dispensing service to 1,400 patients. Our clinical team includes four partners and a diverse multi-disciplinary group, featuring a first contact physiotherapist, mental health liaison worker, social prescriber, and recovery worker. Additionally, we have a robust nursing and healthcare team with expertise in respiratory, diabetic, and women's health.
