Job Overview:
The Care Coordinator's role is pivotal in ensuring the efficiency and quality of our care services. Based in our Colchester Office, the role also involves visiting customers in Suffolk and surrounding areas for assessments and hands-on care, where required.
Key Responsibilities:
• Develop and manage rota/schedule for service users, ensuring their needs are met efficiently.
● Supervise, support, and motivate care staff to ensure they provide compassionate, reliable, and high-quality care services to service users.
● Maintain compliance with regulatory standards and company policies to uphold quality care delivery.
● Assist the Registered Manager in managing and delivering high quality care in compliance with company procedures and policies.
● Picking up new care packages, conducting assessments of new packages to identify the needs, preferences, and any specific requirements of service users.
● Collaborate with service users and their families to develop care plans that address individual needs and preferences while promoting independence and dignity.
● Regularly review and evaluate care plans and medications and making adjustments as needed.
● Facilitate training and development opportunities for care staff to enhance their skills and knowledge.
● Maintain accurate and up-to-date records of customer care, staff schedules, and any incidents or changes in care plans.
● Represent service users to ensure their voices are heard and their needs are met.
Key Requirements:
- Valid driving license with access to own car.
- 1 year experience in domiciliary care
- Minimum qualification of NVQ Level 3 in Health & Social Care or equivalent health care management experience.

Applicants we are looking for:
• Previous experience in a care coordination role or a similar position within the healthcare industry.
• Knowledge of health and social care practices and CQC and other regulations.
• Strong organisational and time-management skills.
• Excellent communicator an active listener and a good team worker.
• Empathetic, dedicated and compassionate for customer and their families.
• Proficiency in using computer software for scheduling and record-keeping.
• Flexibility to work alongside schedule changes including weekends on a pro-rota basis.
• Ability to adapt to new challenges, a learning attitude, while remaining resilient.
• A commitment to promoting dignity, respect, and independence for customers.
• Possess a minimum qualification of NVQ Level 3 in Health & Social Care and be willing to work towards the NVQ Level 5
