Coordination : Collaborate with healthcare providers, community agencies, and insurance companies to arrange for home care services or transfers to other facilities. Support and Counseling : Provide counseling and emotional support to patients and their families during the discharge process. Education : Educate patients and their families about available resources, support systems, and their rights. Documentation : Maintain accurate records of all interactions and discharge plans in the patient’s medical record.
