Coordinating and facilitating interventions to ensure our tenants meet their own aspirations to address their support needs with the objective of promoting greater independence and confidence that will enable them to sustain their tenancy.
Working with tenants who have complex behaviours to ensure that their home is safe, warm and there are mitigations in place to reduce the risk from fire. Working in partnership with the Fire Safety Team and drawing on expertise from the MAGPIE Coordinator.
Delivering interventions to encourage engagement, acting as an ‘agent’ to support tenants to identify barriers to engagement, and supporting them to build relationships with Trivallis, and engage with statutory and non-statutory services that meet their needs and making the appropriate referrals.
Supporting tenants who are the perpetrators of Anti-Social Behaviour to identify and address the root cause of their behaviour, using a strengths-based approach and making referrals to appropriate support agencies where needed, to prevent further instances of ASB.
Managing a caseload and carrying out in-depth needs and risk assessments with those identified as high risk of tenancy failure due to hoarding, ASB or lack of engagement.
Developing a focused framework with individuals to enable and encourage engagement, identifying appropriate solutions for housing related issues with a focus on tenancy sustainability.
Identifying signs and risks associated with domestic abuse, child protection, public protection, and protection of vulnerable adults. Making referrals to external organisations and attend multi-agency meetings as appropriate. Monitoring and reviewing outcomes.
Providing effective signposting and partnership with specialist services where there is a need for issues such as substance misuse, mental health and wellbeing, counselling, or other support-based services such as locality based floating support.
