Magdalen Medical Practice based in Norwich, is seeking to recruit a receptionist/care coordinator to join their busy team on a full or part-time basis. The post-holder will provide extra time, capacity and expertise to our practice team to help us to improve the care provided to our patients. The main duties of the role include: Work collaboratively with other teams and services to maintain an effective and efficient service. Plan and organise work using own initiative, whilst being able to work as a valuable team member. Be able to make referrals to relevant service providers. Coordinate and where required chair MDT meetings, having proactively prepared any actions prior to the meeting and ensure all relevant clinical and non-clinical staff are present. Promote and facilitate integration within the GP and community teams. Provide and receive sensitive information about difficult or complex matters always respecting confidentiality. Coordinate patient care by working closely with GP’s, nurses, and other healthcare professionals. Arrange and manage patient appointments, ensuring efficient use of clinic time and resources. Maintain accurate and up-to-date patient records, including appointment histories and care plans. Monitor patient follow up appointments ensure timely reminders are sent. The ideal applicant will have the following skills, experience and qualifications: Experience of working within healthcare, the voluntary or community sector, to support vulnerable groups. Familiar with local resources and services, including how to access them. Motivated by helping people, with care and empathy. Able to deliver person-centred support in a non-judgmental way. Experience of completing holistic person-centred care planning assessments. Strong listening and communication skills.
