Job summary The post holder is a Clinical Pharmacist (Maternity Cover) who acts within their professional boundaries, supporting and working alongside a team of pharmacists in general practice. In this role they will be supported by a Senior Clinician and Line Manager who will develop, manage and mentor them. Completing the Clinical Pharmacist in Primary Care Pathway and become a prescriber to support the Practice Team. Be responsible for the pro-active care management of patients with complex polypharmacy. Provide specialist expertise in the use of medicines whilst helping to address public health and social care needs of patients and tackle health inequalities;. Provide leadership on person-centred medicines optimisation and quality improvement. Undertake structured medication reviews with patients and produce recommendations for agreement on prescribing changes and monitoring. Work with care home staff to improve safety of medicines ordering and administration; Provide patient facing clinics for those with questions, queries and concerns about their medicines in practices. Review the use of medicines most commonly associated with unplanned hospital admissions and readmissions through audit and individual patient reviews. Reconcile medicines following discharge from hospitals, intermediate care and into care homes, including identifying and rectifying unexplained changes and working with patients and community pharmacists to ensure patients receive the medicines they need. Signposting to other services and/or healthcare professionals where appropriate, while working within scope of practice and limits of competency. To manage repeat prescription requests, resolving queries where possible within scope of practice. To ensure patients have appropriate monitoring tests in place when required. To maintain full and complete records of all patient contacts using appropriate clinical templates and coding. Provide independent information to patients with regard to medicines and prescribing changes, initiating further support from other healthcare professionals where appropriate. To maintain own clinical and professional competence and be responsible for own continuous professional development.
