Job Overview
The South Community Mental Health Service is undergoing an exciting transformation with additional resource allocation to provide ongoing intensive support to the population of Sheffield.
The successful candidate will be supported with their professional and leadership development through dedicated supervision with highly experienced team managers.
Detailed Job Description And Main Responsibilities
The post holder will work within a multi-disciplinary service and core responsibilities will include:
* Acting as a Named Worker
* Comprehensive assessment of new referrals
* Carrying a varied caseload defined by need
* Contributing to a duty system on a rota basis
* Delivering supervision to more junior members of staff
* Contribute to Multidisciplinary service development
Clinical Responsibilities
* Triage and determine urgency of new referrals
* Carry out comprehensive assessments of individuals' health and social care needs
* To assess, evaluate and manage clinical risk
* To formulate a plan of care based on identified need
* Deliver evidence-based interventions as appropriate
* Review effectiveness of care plan at regular intervals
* To work within safeguarding processes
* To maintain accurate computer-held care records

Minimum Requirements
Essential criteria include:
* Relevant Health or Social Care professional qualification (Nursing or Social Work)
* Current registration with NMC, HCPC
* Experience in multi-disciplinary team working
* Extensive experience in mental health in both inpatient and community settings
