Job overview
An exciting opportunity has arisen for experienced individuals to join our Integrated Discharge Team as a Band 7 Therapist Lead. The position is an ideal opportunity for Physiotherapists or Occupational Therapists to shape this new service for our patients. Are you passionate about supporting patients to discharge home as soon as they are able? You will be working and leading a team that is motivated to ensure that patients should be returning home as soon as they are able. The ethos of the team is 'Why not home, why not today'.
The successful applicants will be key members of the Care Transfer Hub with a multidisciplinary team of health and social care professionals.
Main duties of the job
As a senior clinician within the Care Transfer Hub (CTH) multi-disciplinary team you will work with a “home first” philosophy. You will be an advocate for the patient in determining the right pathway for them as individuals following a discharge to assess model. You will be working in collaboration with other health and social care professionals in an integrated way providing autonomous clinical, professional decision-making to support the planning of complex discharges. The post holder will develop the ethos of multi-disciplinary and seamless services within Shropshire Telford and Wrekin’s acute and/or community hospitals ensuring that discharge planning is delivered to the highest possible standard.
The post holder will be working flexibly across both acute hospital sites and may be required to work in community settings as part of their role and participate in 7 day working when appropriate.

Person specification
Training and Qualifications
* Qualified to degree level or equivalent Evidence of post-registration qualifications/professional development in related health field
* Therapy based Degree/Diploma
* Membership of relevant professional body
* Registered with Health Professional Council
* Experience of working with occupational therapy/physiotherapy client groups plus further theoretical study and experience within a community environment to master’s level equivalent.
Experience
* Proven ability of complex case management
* Extensive experience as a senior clinician/practitioner
* Evidence of effective staff and resource management
* Understanding of common medical and surgical conditions, care of the elderly, neurology, trauma and orthopaedics
* Previous experience in a physical setting.
* Post-registration experience across either Community or Acute or Social Care settings in a Senior Therapist level post.
* Management or supervision experience such as appraisals of staff and day to organisation and monitoring of staff workload
* Multi-disciplinary team working
Knowledge and Skills
* Demonstrable evidence of continuing professional development and a drive towards continuing developing knowledge and skills in area specialism
* Ability to manage complex caseload
* Ability to drive high level multi-disciplinary practice to maximum potential
* Ability to engage in audit/research process in order to influence care according to best practice
* Knowledge of relevant national policies pertaining to profession
* Knowledge of up-to-date practice using research and evidence-based learning
* Thorough knowledge of Discharge To Assess Pathways and The Hospital Discharge Service: Policy and Operating Model
* Good understanding of aims and priorities of Intermediate Care and hospital discharge

* Must be able to meet the transport requirements of the post
