Job summary Dementia Intensive Support Team - Senior DIST Practitioner Band 6 (Mental Health Nurse/OT/Social Worker) DISTe is a community team covering East Suffolk, driving is essential, we have an office base at Walker Close in Ipswich and often work from home. DISTe aim to prevent avoidable/inappropriate admissions to acute hospitals and to support with discharges. The team supports people with dementia or suspected dementia who are at risk of admissions caused by Biological and Psychological Symptoms of Dementia such depression, anxiety or challenging behaviour. The service shall focus on: Maintaining independence for people with dementia People admitted into acute hospitals with anticipated short lengths of stay in order to reduce the number of such stays becoming long term, and to reduce the incidence of readmission Reducing the premature need for residential care in partnership with social care We are looking for energised individuals who will be excited to be part of something new and very special, as our service specification is currently under review. If you want to make a difference in people's lives and change it for the better than we want you to be part of our fantastic team Main duties of the job Duties will include, but not exclusive to: Assessments of People Living with Dementia or suspected Dementia Caseload holding Multi-agency working; including family, acute hospital, primary care sector, social care, care providers and voluntary services Triage of referrals Working with Patients and Carers on a one to one basis Supporting the Duty role within the Team The post holder will undertake the planning and delivery of direct patient care with appropriate supervision, ensuring continuity of practice and working within a multi-disciplinary and multiagency environment. Managing a caseload and the provision of Mental Health Care services to people with mental health care needs. Support and advising to other health care professionals. To support aspects of service development Provide an appropriate learning environment for relevant UEA students. To be accountable for own professional actions. Participate in mental health promotion. Provide the appropriate interventions to patients with mental illness (incorporating NICE Guidelines). Effective personal planning and organisation of daily work load. Flexible working and manage own working time effectively and accordingly to service demands. Practice autonomously in making clinical judgements, identifying alternative courses of action, managing and providing a high quality of care. Maintain respectful, effective professional interpersonal relationships with others.
