Our pharmacists are valued members of the inpatient multidisciplinary teams and the service is currently in a process of developing a strategy to expand our clinical services and deliver a high-quality medicines optimisation and management service across all trust services.
The Trust has pharmacy teams based in Barnsley, Calderdale, Dewsbury, Halifax, Huddersfield, and Wakefield.
Main duties of the job
Our clinical pharmacists provide medicines optimisation services to our inpatient wards and community mental health teams, having direct patient contact where possible.
We provide medicines information and clinical review of more complex cases for clinical colleagues.
We have just completed EPMA roll out to all our mental health inpatient wards.
You would be supported and working within our specialist teams of pharmacists and pharmacy technicians.
Other work may include contributing to audit and assurance programmes, education and training of pharmacy colleagues and other professionals or supporting production of clinical guidance.
Job responsibilities
1. To participate in the provision of a safe and effective specialist clinical pharmacy service to designated wards within SWYPFT.
2. To assist Lead Pharmacist, Senior Clinical Pharmacist in the overall provision of pharmacy services to units/teams within SWYPFT.
3. To deputise for Lead Pharmacist, Senior Clinical Pharmacist as appropriate.
ROLE OF YOUR DEPARTMENT
1. Maintain and develop efficient and cost-effective systems for procurement, compounding, preparation and distribution of medicinal products, which reflect the needs of individual patients, patient groups as determined by the Trust.
2. To ensure compliance with the current law relating to medicines and medicinal products.
3. To provide professional advice to relevant health care professionals and patients regarding the safe, cost-effective use of medicines and medical products.
4. To identify and ensure the operation of quality control mechanisms that meet formal controls required by the medicines agencies.
5. Identify and address clinical risk issues by applying principles of clinical governance, Medicines Management and controls assurance.

* Bachelor/Master's degree in Pharmacy
* GPhC registration as a Pharmacist

* Supplementary prescribing qualification
