Urgent Community Response Support Worker

Duties will include support with and provision of personal care, clinical duties where delegated, monitoring the patient's condition and working with health and social care professionals to ensure that patients are safe and well at home. This role includes medication prompting and administration, facilitating adequate hydration and nutrition and supporting with goal progressions as part of the patient's journey to rehabilitation/recovery. You will need great interpersonal and communication skills and be able to connect with individuals with varying levels of understanding.
You must have a full UK driver's licence with access to your own vehicle (with appropriate business insurance) as you will be travelling to patients' homes to care for them. Expenses incurred for work travel will be paid at 59p for the first 3,500 miles.

Job responsibilities
We have opportunities to join the Royal Devon University Healthcare NHS Foundation Trust Urgent Community Response teams of front-line staff, providing support to patients at home. This includes following discharge from hospital, enabling acutely unwell patients to remain in their own home and also helping with end-of-life care.
Previous community experience is desired but not essential as the teams will give you the support and guidance you need to excel. You will join a dynamic multi-disciplinary team, working with Health and Social Care Teams to respond to urgent situations, enabling people to stay in their own homes or ensuring a timely discharge from Acute and Community Hospitals.
You will be provided with a comprehensive induction programme which will include support to complete the National Care Certificate.
Successful candidates will have a Level 3 Diploma / NVQ 3 in Health and Social Care or equivalent and ideally have experience of working with older people in a community setting. You will also need to be competent with IT as electronic recording is an essential part of the role. As this is a community-based position, a driving license is essential as is access to a vehicle.

Urgent Community Response Support Workers are lone workers within the community, although support is always available.
Working Pattern: The service operates on a 7-day rota system between 07:30-22:00, including evenings, weekends and Bank Holidays.
