An exciting opportunity for an organised and highly motivated Clinical Pharmacist to join our friendly and helpful team at Mayfield Medical Centre. We have a multidisciplinary team consisting of advanced health professionals including Paramedics, First Contact Physiotherapists, Health Care Assistants, Practice Nurses, Nursing Associates, Mental Health Workers, and Salaried GPs.
Salus Medical Services hold the contract for Mayfield Medical Centre, which is a new and exciting change to how practices are run. Rather than having a Partnership to contend with, as a non-for-profit company, all the money the practice earns is reinvested back into the practice. We facilitate an innovative and friendly atmosphere, where ideas are encouraged, everyone is welcome, and everyone works as part of a team. Our ultimate vision is to work within the local community to offer preventative style healthcare. The practice is located in north Farnborough and has a list size of 9500 patients. We would be very excited to welcome a new Pharmacy Technician as part of this exciting new vision.
Main duties of the job
You will work closely with the pharmacist and prescription team to optimise the efficiency with which the team runs, aiding in the processing of repeat prescriptions, identifying and streamlining the medication review process, assisting with improving medicines optimisation, and carrying out medication audits.
Other tasks will include but not be limited to acting upon compliance issues, screening and facilitating the completion or signposting of EDTs, liaising with external stakeholders, accurate recording of patient documentation or information into care records, keeping the practice up to date with prescribing information and data, and others.
Job responsibilities
* Support and guide the Receptionist Team in the processing of repeat prescription requests and medication queries
* Actively encourage patients to reconcile medications to facilitate monthly collections and support patients with their requirements
* Act upon compliance issues, liaising with the appropriate clinician
* Identify patients for medication review and support the multidisciplinary team to improve medication reviews and maintain an accurate recall system
* Effectively liaise with external services i.e. district nurses to ensure medications are arranged for housebound patients
* Input data into the patients' healthcare records as necessary
* Record and report adverse effects accurately and appropriately, informing the practice Clinical Pharmacist / registered GP immediately
* Use local protocols to ensure correct monitoring of high-risk medications
* Use prescribing data to identify priorities to improve prescribing and patient care
* Access clinical IT systems to document activities and run searches
* Run audits when necessary
* Promote medicines optimisation to improve patient outcomes and to reduce waste
* Develop relationships with the wider multidisciplinary team and promote networking
* Promote the use of ERD and EPS within the practice
