An exciting opportunity has arisen for a highly specialist OT to be part of our Community Rehabilitation team within Ealing Short Term Rehabilitation.
You will be able to provide decisive and effective therapy input and will demonstrate leadership qualities, excellent communication skills and a flexible approach when working under pressure. You will be passionate about the value of community rehabilitation and preventing hospital admissions.
The successful candidate will be an experienced clinical leader with the ability to motivate and inspire others. They will have a proven track record of providing exceptional care to service users and carers while supporting staff to deliver an excellent service.

Main duties of the job
* To be professionally and legally responsible and accountable for all aspects of own work including the management of patients.
* To undertake highly specialist assessment of patients, interpreting complex findings, to enable an appropriate treatment plan to be formulated, delivered and progressed for a wide range of complex conditions, using highly developed clinical reasoning and skilful therapeutic handling developed from specialist knowledge and using extensive practical experience to guide the process.
* To support and supervise junior Occupational Therapists, Rehabilitation Assistants, students on placement and locum therapists.
* To assess, prescribe, issue, and instruct patients in the safe use of a variety of equipment including a range of functional aids, manual handling aids and mobility equipment.
* To undertake assessment of need for postural support/seating and work closely with wheelchair services and specialist seating services.
* To undertake specialist occupational therapy assessment and treatment as an autonomous practitioner.

* To assess, diagnose and develop individual care/treatment plans for a range of conditions, some of which will be highly complex or will be for patients with complex clinical and social needs, and to be responsible for reassessing patients care as treatment progress and alter treatment programme as appropriate. To evaluate and develop treatment programmes.
* To advise and teach patients, carers and other professionals to participate in occupational therapy and intervention plans for the individuals.
* To ensure a high standard of clinical and professional practice.
* To undertake specialist occupational therapy assessment and treatment as an autonomous practitioner.

Qualifications and Experience
* Degree / Diploma Occupational therapy
* Health Care Professionals Council Registration
* Relevant Professional Body Membership
* Evidence of continued professional development
* Post-graduate Qualification relevant to the healthcare environment.

Experience

* Experience in managing complex individual & team’s caseload of patients
* Experience of supervising and appraisals- junior colleagues & students
* Experience of performance management & dealing with complaints
* Previous community therapist experience in NHS or similar healthcare economy
* Experience of leading a team
* Experience of case management including multi-agency working
Knowledge
* Knowledge of developments within NHS and specialist occupational therapy professional knowledge
* Knowledge of long-term conditions including end of life care and the have the drive toward care for these patients
* Knowledge and understanding of the NHS modernisation agenda and the implications for community services
