Job summary An exciting opportunity has arisen to work within a well-established Community Pulmonary Rehabilitation Team here in Shropshire, Telford and Wrekin supporting patients to make positive life choices that enhance their lives and health. As part of the Pulmonary Rehabilitation team you will be expected to work independently in liaison with the physiotherapists and nurses, to support patients in a community setting. You will work alongside and build relationships with the community respiratory nursing teams, local GP surgeries, secondary care respiratory colleagues and community inter disciplinary teams, delivering high quality care across community settings.
