The post holder will be required to ensure that all data relating to the monitoring of the 18-week pathway for patients is collated and processed in accordance with national and local timescales for submission of data, providing timely information to managers to ensure that national targets are not breached.
Working to ensure completeness and quality of data, the post holder will engage and work alongside Team Managers and will liaise with medical staff, nursing staff, healthcare professionals, Business Intelligence Team and other related staff both within and external to the trust using all networks and resources to expedite the patient pathway.
Main Duties of the Job
To liaise with secretarial, nursing and clinical teams to ensure investigations are booked in a timely manner and to re-schedule investigations and treatments to facilitate the achievement of national targets, including forewarning clinicians and managers of the risks of a forthcoming breach in targets so that measures are put in place to avoid breaches.
Work proactively to identify risks and work with the teams to take corrective action to ensure that targets are met.
Use all related trust information systems (i.e. EPR, Pathology, Radiology and Theatre systems) for the timely capture of data and maintaining data quality.
Liaise with system developers and key stakeholders on specific requirements relating to patient pathway monitoring, and develop an information and support network to ensure the spread of good practice across the trust.
Provide administrative support at meetings relating to the 18-week pathways, providing statistics and information where appropriate.
To work with Team Managers as an integral part of the team ensuring that information is available and presented at key meetings regarding obvious system blockages.

Job Responsibilities
Main Duties & Responsibilities
Effectively coordinate the tracking of the administrative pathway of patients, providing links across specialities and clinical teams, developing systems to ensure that all steps on the patient pathway are booked and recorded appropriately with rapid communication and information exchange between all relevant staff.
To set up systems to record and monitor the patient journey times from PTL lists and work with Medical Secretaries and their manager supporting them to ensure wait times are in line with the 18-week performance target.
Monitor and manage waiting times by proactively identifying risks and taking corrective actions to ensure that the waiting time targets are not breached.
Timely and accurately record outpatient procedure coding on Medway.
Support with the management and validation of outpatient waiting lists and escalation of long waiters.
Person Specification
* Demonstrable experience of data collection, validation, analysis and reporting
* Working knowledge of 18-week pathway and Patient Choice Agenda
* Full working knowledge of EPR across Medway, iPortal, and other clinical systems
* Previous experience coordinating or managing waiting lists
