Job summary The post holder will support the Pharmaceutical Service and the Medicines Management agenda in the UHB. They will be responsible for: Assisting in the delivery of clinical pharmacy services to the medical wards to ensure safe, therapeutic and cost-effective use of medications. Ensuring patients are prioritised appropriately and high-risk patients are identified and referred to the appropriate member of the multi-disciplinary team or managed by the post holder. Continuing the development of pharmacy services provided across the medical wards. Main duties of the job The post holder will support the Pharmaceutical Service and the Medicines Management agenda in the UHB. The post holder will sit on the Medicines Management panel, assisting in the training and assessing of technicians, trainee pharmacists and new pharmacists in the process of ward-based working, including medicines reconciliation and discharge process. In addition, they will support others in developing clinical prioritisation skills. The post holder will dispense, undertake technical check and issues prescriptions, providing specialist advice and information on medicines to patients, relatives, carers and nursing staff.
