Key Responsibilities:
1. Assessments: Carry out comprehensive social work assessments for individuals, families, and carers, identifying needs, risks, and safeguarding concerns. Ensure assessments are thorough, evidence-based, and completed in accordance with statutory requirements and best practice guidelines.
2. Care Planning: Develop, implement, and review person-centered care plans to support individuals and families in overcoming challenges. Ensure care plans are realistic, achievable, and tailored to each individual's needs and circumstances.
3. Advocacy and Support: Act as an advocate for service users, ensuring they have access to the services and support they need. Provide information and advice on rights, available services, benefits, and community resources.
4. Safeguarding: Identify safeguarding concerns and take appropriate action in line with safeguarding procedures. Work closely with safeguarding teams and other relevant authorities to protect vulnerable individuals from harm and neglect.
5. Multidisciplinary Collaboration: Work collaboratively with health, social care, and other professionals to develop and implement holistic care plans. Participate in team meetings, case discussions, and care reviews to ensure the best outcomes for service users.
6. Crisis Intervention: Provide support during crises, offering emotional and practical assistance. Offer timely interventions to prevent unnecessary hospital admissions or ensure safe discharge from services.
7. Case Management: Manage a caseload of individuals, ensuring that all interventions and progress are recorded, reviewed, and updated regularly. Provide continuity of care and coordinate services to meet individual needs.
