About the Team: The successful applicant will become part of an experienced and motivated Mental Health Social Care Management Team. As a Delivery Lead you will support our social care Team Managers in the South of the County who oversee the work of the dementia, older adult and CMHT social care pathways, as well as our specialist Mental Health carers service. This Delivery Lead role is a unique opportunity to build upon our successful relationship with Coventry and Warwickshire Partnership Trust (CWPT) where we are seconded to work under a successful section 75 agreement. As a Delivery Lead you will work closely with CWPT partners to identify and address social care needs both for new referrals, as part of the triage process, and for those already open to services. You will also have a key link role in facilitating the discharge of psychiatric inpatients by ensuring social care needs are identified as early as possible with a view to accessing support from either our urgent or community social care pathway. You will be able to demonstrate excellent organisational skills, a head for complexity, and a desire to improve the outcomes and wellbeing of those you work with. Above all we are looking for inspirational leadership from someone truly motivated to improve the outcomes of the people we work with. If you are a qualified AMHP you will spend one day a month on our countywide AMHP rota assessing people of all ages who are experiencing a mental health crisis.
