Pembrokeshire County Council are seeking experienced social workers to work within our Locality Team.

Responsibilities:
* To assess, and/or co-ordinate the assessments of the strengths and needs of users and their primary carers using the established assessment format, together with the department’s criteria for service provision within the legislative and regulatory framework
* Develop and cost packages of care which are flexible, cost effective and responsive to the changing needs of the individual. Promote independence, self-determination and the maximum choice through access to a range of services provided by the independent and statutory sectors.
* To undertake specialist social work care planning including the provision of direct specialist social work interventions.
* To contribute to the management of the adult care budget and the maintenance of expenditure within set budget limits.
What we are looking for in a Social Worker:
* Social Work England registration
* Eligible to work in the UK
* Hold a full UK licence
* Post-qualified experience
* Degree level or equivalent in Social Work
