Job summary An exciting opportunity has arisen for A Registered Community Mental Health B6 Nurse to join our dedicated Memory Team at Monarch House in Scunthorpe. You may be currently working within other areas of Older Adult or Adult Mental Health; however, this would give you a fantastic opportunity to join a very Specialised Experienced Older Adult Mental Health Team in a Community Setting. Currently the Memory Team has several experienced Band 6 Nurses who carry out initial assessments, memory medication, antipsychotic and diagnostic reviews/appointments, as well as Section 117 reviews. We are a very busy dedicated team who support one another through Clinical/Managerial Supervision, Clinical Forums and Multidisciplinary Team working. We work collaboratively with both our Consultant Psychiatrists, Nurse Consultant and Clinical Lead Nurse to provide safe and holistic care to our patient group. The post holder will be supported to develop your nursing skills in delivering nursing interventions, with the scope of working towards achieving a Non-Medical Prescriber Role within the team. Main duties of the job To provide a high quality, accessible service for patients in the North Lincs area, who are experiencing memory problems. To complete initial assessments, cognitive testing, and monitoring of the needs of clients experiencing memory problems. To provide ongoing education/guidance/support to clients and carers both during and after assessment. To work as a supplementary/independent prescriber. To provide a holistic patient assessment and structured carer interview. Responsible for completing specialist initial assessment and interpretation of results to provide a baseline for future treatment/care. To work autonomously and show initiative when planning clients care. To deliver diagnosis and offer expert advice regarding the diagnosis and treatment and provide education/support to both clients and carers.
