You will be working in a friendly and supportive environment during what is a very exciting period as we continue to shape and develop the model of care by empowering nurses to facilitate early decision-making in the safe discharge of our patients. You will work closely with all members of the MDT to deliver patient centred care. We are passionate in empowering and helping all patients to gain/regain their skills and confidence and our focus is always to achieve safe, successful and effective discharges. We care for our patients 7 days a week, 24 hours a day so you will need to have a flexible approach to take part in a full shift system including nights and weekend working, in some areas on a 12-hour shift system or on a three-shift system.
