Critical Care Outreach Nurse

Permanent, Full time, 37.5 hours per week

The Critical Care Outreach Nurse is responsible for the delivery of the highest standards of care and service to inpatients located in any area of the hospital, aiming to identify deteriorating patients in the wards, providing them with the necessary care to prevent or ensure admission to intensive care in a timely manner. The outreach nurse will also be following up all Intensive Care Unit patients discharged to the ward as well as the continuous development of the ward staff through clinical support and training

While supporting the wards and other inpatient areas, you will come across a wide range of cases which include Colorectal, Orthopaedics, Cardiothoracic, Lung surgery, Renal Transplants, Vascular, Liver Robotics, Neuro, Maxillofacial, Medical and Oncology patients. Your duties will involve but not limited to:
1. Independently act as a specialist resource to provide critical care expertise to the multidisciplinary team, irrespective of patients’ clinical speciality or presenting condition, acting as an emergency resource via bleep system to calls for support to Level 1 adult patients.
2. Make suggestions for patient management through analysis of available information and initiate referrals to multidisciplinary specialists as judged appropriate.
3. Lead on team specific audits (EWS, Sepsis and AKI) and shares results with appropriate teams.
4. Provide formal teaching during hospital induction on the role of Outreach, as well as on the delivery of the Deteriorating patient programme.
Key Skills / Qualifications
5. An active Adult General Adult NMC pin
6. Experience as Senior Staff Nurse or above in Critical Care
7. Outreach experience is advantageous.
8. Team player abilities
9. Proven experience in audit, policy development and implementation
10. Successful completion of the ICU & Mentorship Course
