Would you like to work within a supportive and compassionate team where we work openly, combining our strengths, helping us to achieve more together?Our Hospital discharge team are there to provide support to people with a diagnosis of Dementia with a successful discharge from hospital to ensure that they have the right support to return to their homes safely and with confidence. This role will support people and help provide the tools and knowledge to make informed decisions about their future. This particular role will be based in Basildon Hospital and will require a large amount of walking throughout the different departments. There is no direct care element to the Dementia Adviser role, however we work passionately to provide advice, support and guidance to people affected by dementia by working collaboratively with teams within the hospital. About you Ability and interest in supporting people to maintain their independence, improve their sense of well-being, and help them take more control of their lives. Understanding of dementia and the needs of those living with dementia and their carers would be an advantage but if you have had experience supporting with other diagnosis or support requirements this would be taken into consideration. Experience with communicating with a wide range of people, adapting your approach to ensure understanding at all levels. Able to network effectively and collaborating with other professionals to achieve a positive outcome for people. Good IT skills, experience working with databases and virtual meeting software (Teams / Zoom). The ability to travel across County of Essex independently.
