We are looking for enthusiastic Social Work professionals across Devon who can evidence a commitment to strengths-based practice, a real pride in their profession and passion for delivering Social Work and Social Care interventions as part of a varied and skilled MDT., The role will involve working as part of a wider multi-disciplinary team (MDT) supporting people through the Rehab pathway in both inpatient community settings upon discharge, using your expert Social Work skills and knowledge in assessment, support planning and commissioning in line with the expectations of the Care Act 2014 and S117 aftercare planning.
You will be an experienced and qualified Social Worker (CQSW, DipSW, BA Hons, MA or equivalent) who is registered to practice with Social Work England. You will have completed the Assessed First Year in (post-qualifying) Social Work practice, if appropriate, and can demonstrate a commitment to ongoing professional development as we are committed to supporting you to look at post-qualifying opportunities during your career with us., We will support you to learn the skills you need to thrive in the role.
