An exciting opportunity has arisen for an experienced and enthusiastic Health Care Assistant to develop their knowledge and skills to coordinate patients safely through the Discharge process ensuring a smooth transition from hospital to home.
The Discharge Lounge is a 5 day week service, Monday to Friday operating between 8-8pm. Our aim is to provide a comfortable, safe and efficient environment for our patients transferring from hospital to home, liaising closely with Packages of Care, pharmacy and Transport Services. We provide support to ward areas, patient flow and to the patients within the lounge until they are discharged safely home.
We are looking for a motivated and self-driven individual whose priority is to deliver high quality patient care and service throughout the Discharge Process.
The successful post holder will be calm, friendly, enthusiastic and adaptable to change as well as possessing excellent organisational and communication skills. They must be able to demonstrate flexibility to meet these service demands, work well under pressure and manage challenging situations while having the creativity and drive to develop the service.
Main duties of the job
* To coordinate the day-to-day process of patient discharge.
* To advise and assist ward teams with simple and complex discharges.
* Proactively encourage and facilitate timely and seamless discharge/transfer to another care setting via the discharge lounge.
* To proactively liaise with the relevant services when the discharge is dependent on a specific action, i.e. Transport, QEF, NEAS, Pharmacy, Packages of care and Nursing Homes.
* As the central point of contact for discharge lounge provide regular discharge updates to patients, family members, ward teams, internal and external support services/agencies.
* Provide patients, their families and carers with details of who to contact about medicine and equipment problems that occur in the 24 hours after discharge.
* Complete any admin and data collection relating to the ward including f&f tests, Nerve centre, Discharge database to include statistical data.
* Promote shared understanding of discharge processes and requirements to wider team(s).
* Support Trust-wide service improvement initiatives such as implementation of the Discharge Lounge pathway.
