Job Description:
We are seeking a highly motivated and experienced Band 6 Physiotherapist to join our Allied Health Professionals team. This role focuses on delivering high-quality physiotherapy care to housebound patients in the community, with a particular emphasis on fall prevention and management.
Key Responsibilities:
1. Conduct community visits to assess and treat housebound patients, delivering tailored physiotherapy interventions.
2. Triage patients for the Falls Practitioner team, prioritizing urgent or high-risk cases for referral to the BHR Falls Service.
3. Develop and implement individualised care plans to reduce the risk of falls and improve mobility and independence.
4. Provide education and support to patients, families, and caregivers on fall prevention strategies.
5. Collaborate with multidisciplinary teams to ensure a holistic approach to patient care.
6. Maintain accurate documentation in line with professional and organisational standards.
7. Participate in service development and quality improvement initiatives within the community and falls service.
Requirements:
1. Experience working with elderly patients, in community settings, and managing falls cases.
2. Strong clinical assessment and decision-making skills.
3. Excellent communication and teamwork abilities.
4. A valid UK driving licence and access to a car, as the role requires travel within the community.
