Job summary The Community Mental Health Services for Older People are looking to recruit an experienced Mental Health Nurse to join our Team. The Crisis Function team is made up of a already established multi-disciplinary team. The Post is based in East Hertfordshire, which includes Welwyn Garden City, Cheshunt, and Bishops Stortford. The team provides the crisis function to our community team and is responsible for the support and treatment of older adults who are experiencing a mental health crisis - with people that have either a functional or organic condition. The team also has responsibility for assessment and gatekeeping people who may require admission to a mental health inpatient unit.Mental Health Services for Older People are comprised of a community team, a crisis function, a social work function and a therapy function that work in an interlinked way to provide care to people. As a professional you will have regular supervision and a Personal Development Performance (PDP) that aims to work with your strengths and to develop your skills. The crisis function works across 7 days, providing a service between 09:00 and 21:00hrs on weekdays and between 11:00 and 19:00hrs on weekends. The post holder will therefore be required to work within the shift pattern. The post holder is required to take responsibility for the assessment of care needs, the development, implementation and evaluation of programmes of care and the setting of standards of care and management of a defined caseload, including liaison with other agencies and, where appropriate, the supervision, development and teaching of staff and/or students. The post holder will act as care coordinator to a case load of service users, ensuring the care plan is met at all times. To work as a member of the multi-disciplinary team and complete initial assessments for the community team as required.
