The role of a Pathway Management Co-ordinator is to proactively monitor and track patient pathways to ensure the patient journeys are accurate and in line with national and local defined targets. This includes identifying and addressing potential breaches and escalating to the relevant teams working alongside all clinical delivery groups and Service managers. Monitoring and tracking of patient pathways against operational targets including RTT, Inpatients, cancelled operations, emergency department attendances, referral handling, advice & response and diagnostics. As an extension of pathway management, co-ordinators are also required to contact patient cohorts to ensure accurate recording of information and waiting list status.
Main duties of the job
The Operational Performance Team is responsible for tracking and monitoring of organisational and nationally mandated performance targets to support service delivery. In addition to maintaining this accountability, the team holds several centralised functions to support operational teams and delivery including:
1. Validation, escalation and review of the patient pathways specifically for RTT via the PTL
2. Validation, escalation and review of patient pathways against operational standards and processes
3. Pathway validation and review of waiting lists via direct patient contact

Person Specification
Qualifications
* Good General Education (e.g., GCSE English and Maths A-C)
* Business Administration NVQ level 3 or equivalent experience in an administrative environment
Experience
* Experience working in an acute Trust
* Understand confidentiality and apply the principles in everyday working practice
* Experience of dealing with the Public/Customer service experience
* Proficient in the use of Microsoft systems i.e., Office, Excel, Word.
