Advanced Nurse Practitioner - Langley Corner Surgery
Alliance for Better Care are advertising on behalf of Langley Corner Surgery - Crawley Care Collaborative PCN, who are looking for an experienced Advanced Nurse Practitioner.
Whilst General Practice has some challenging times ahead, this role provides an exciting and innovative opportunity for someone who wants to make a real difference in the communities they serve. The successful candidate will be provided with excellent support, the opportunity for development in the role, and will be clinically supervised by a General Practitioner.
Main duties of the job
The post holder is an experienced nurse who, acting within their professional boundaries, will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care. They will demonstrate safe, clinical decision-making and expert care, including assessment and diagnostic skills, for patients within the general practice. The post holder will demonstrate critical thinking in the clinical decision-making process. They will work collaboratively with the general practice teams to meet the needs of patients, supporting the delivery of policy and procedures, and providing nurse leadership as required. In order to work at this level NMC requirements for advanced practice must be met.

Crawley Care Collaborative Primary Care Network is an NHS Collaboration between four GP Practices - Bridge Medical Centre, Langley Corner Surgery, Leacroft Medical Practice and Southgate Medical Group - working together to provide enhanced access services.
Our surgery teams work closely, sharing expertise and resources to develop new services. Our vision is to continue to improve the quality of care that we provide in alignment with the needs of our patient population.
Our Primary Care Network builds on the existing primary care services and enables a greater provision of proactive, personalised and more integrated health and social care. We are supported by practitioners in additional roles who allow us to create bespoke multi-disciplinary teams based on the needs of our local population.

Key Responsibilities
Clinical Practice
1. Assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis.
2. Clinically examine and assess patient needs from a physiological and psychological perspective.
3. Diagnose and manage both acute and chronic conditions, integrating both drug- and non-drug-based treatment methods into a management plan.
4. Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national, local and practice protocols, and within scope of practice.
5. Support patients to adopt health promotion strategies that promote healthy lifestyles, and apply principles of self-care.
Communication
1. Utilise and demonstrate sensitive communication styles, to ensure patients are fully informed and consent to treatment.
2. Communicate effectively with patients and carers, recognising the need for alternative methods of communication.
3. Act as an advocate for patients and colleagues.
Leadership, Personal and People Development
1. Take responsibility for own learning and performance including participating in clinical supervision and acting as a positive role model.
2. Support staff development in order to maximise potential.
Management of Risk
1. Manage and assess risk within the areas of responsibility, ensuring adequate measures are in place to protect staff and patients.
2. Monitor work areas and practices to ensure they are safe and free from hazards.
Person Specification
* Commitment to reducing health inequalities and proactively working to reach people from all communities.
* Able to recognise personal limitations and refer to more appropriate colleague(s) when needed.
* Access to own transport and ability to travel across the locality on a regular basis.
Qualifications
* Registered first level nurse MSc or equivalent.
* Clinical supervision training and experience.
Experience
* Recent primary and community nursing experience.
* Nurse-led management of minor illness, minor ailments and injuries.
Knowledge and Skills
* Advanced clinical practice skills.
* Management of patients with long-term conditions.

£49,419.20 to £55,654.05 a year Dependent on experience
