Job summary You will ensure that the medicine management system for the practice is implemented and working well. This includes ensuring cost-effective prescribing, supporting colleagues and patients with medicine queries, and issuing prescriptions as an independent prescriber. You will be guided by targets and requirements set by the NHS locally and nationally for our practice. Main duties of the job Review and action safety alerts from the MHRA or other bodies, for the practice Carry out medication reviews for specific groups of patients, including those on high-risk medicines Reconcile medicines from hospital discharges or specialist appointments, and for newly registered patients Review and action any directives from BSOL ICB with regards to medicines management requests or requirements Encourage the practice into prescribing in a cost-effective manner, including use of the BSOL Formulary Be a reference point for colleagues for questions regarding medications Perform audits on medicines topics as directed by the practice Update colleagues on the latest guidance regarding medicines, including from e.g. NICE Issue repeat medicines and reauthorise medicines as appropriate Assist with compliance with national targets, including e.g. QOF About us The Limes Medical Centre is looking for a clincial pharmacist that is comfortable working within a close team, to ensure our service provides care that is safe and effective.
