Job summary We are seeking to recruit an enthusiastic and highly motivated Community Occupational Therapist to work within our Henderson Rehabilitation Unit team. Within this opportunity, you will provide management of people with long term conditions, including the promotion of health and prevention of secondary complications. We actively promote a multi-disciplinary model within the units to ensure the person we are supporting is kept at the heart of every decision. You will work closely with case managers and/or social care providers to facilitate safe and timely discharges, while being part of an integrated, supportive multi-disciplinary team (MDT). If you are passionate about delivering exceptional community-based therapy, improving quality of life for service-users and committed to rehabilitation in a home environment, we would love to hear from you Working Pattern Monday to Wednesday, 8:30 to 16:30 Main duties of the job With the support of senior staff you will be responsible for the initial holistic assessment, implementation and evaluation of person centred, goal-based treatment plans for patients who are acutely and chronically unwell; often with complex needs arising from the frail elderly cohort. You will need to able to use clinical reasoning skills to develop, facilitate or supervise the running of patient groups using occupational therapy skills for physical, cognitive and social rehabilitation purposes promoting high quality and harm free care. Collaborate and negotiate with service users, carers and family in identifying appropriate and achievable goals as part of the overall care plan, which will maximise quality of life and independence, and facilitate appropriate referrals to other services or investigations based on your assessment to ensure holistic patient care. Work within a multi-disciplinary team, to provide integrated skills and sharing knowledge providing holistic and specialist skills supported by a robust competency framework. To assess and arrange the provision of equipment and/or adaptations from appropriate agencies ensuring this is provided in good working order and fitted correctly; reporting any issues appropriately.
