Job summary The post holder will be responsible for creating a comprehensive and collaborative approach of care for high intensity users of the Kirklees Place using Calderdale & Huddersfield NHS Trust, other urgent and emergency care providers and other Kirklees Place stakeholders. To support patients in A&E and the community in accordance with up-to-date clinical guidelines and best practice. To work without direct supervision, but work with the support from the service manager. To use specialist knowledge and skills to connect with others, to be able to change the behaviours of people with complex needs, to draw on the involvement and valuable skills of those around them, and develop collaborative care plans with Service Users and other agencies. To develop the service, working with internal and external stakeholders to establish robust processes for identifying high intensity users of the Trust's services. They will develop and improve the care planning process and monitor the impact of intervention for patients, the Trust and external agencies and providers.
