Job summary We have an opportunity for a proactive Care Coordinator to join the organisation on a fixed term basis. TOG Mind is a fast-paced charity delivering a wide variety of mental health and emotional wellbeing services. In your role as a CYP Population Health Care Coordinator you will be part of an innovative population health management programme focused on providing more proactive identification and support to children and young people (aged 5-17) at risk of developing mental health condition. You will work across the West neighbourhood from community settings, using a data led approach to identify a cohort of young people to be supported through the offer. You will be responsible for making initial contact, coordinating appointments and being a first point of contact to regularly review care planning. You will work as part of the PHM team and be supported by Tameside Oldham and Glossop Minds dedicated children, young people and familys directorate and you will also connect with our Adults Navigator who supports the West district Main duties of the job You will be responsible for working with the PHM Navigator in the identification, engagement and case management of at risk CYP, supported by data. You will be responsible for making initial contact with prospective families, clearly communicating the programme and triaging for suitabilty. You will coordinate appointments with the Population Health Navigator and will be a main point of contact throughout the programme for young people and their families. You will complete regular reviews with to ensure that the care plan devised by the PHM Navigator is meeting their needs. You will work from community bases across Oldham West and from the Mind office in Oldham as well as building strong, trusted relationships with the children and families this model seeks to support. You will work with the PHM Coordinator to monitor and track the progress of CYP, the effectiveness of interventions and the exploration of mechanisms to support new ways of working.
