Job summary An exciting opportunity has arisen within the Review Team The Review Team is a multi disciplinary with team members from backgrounds that incorporate Social work, registered mental health Nursing and Occupational therapy, who review individuals who have a funded package of care. Where appropriate the team will look at lesser supportive packages to enable increased independence. Main duties of the job Completing reviews of packages of care funded by Manchester City Council and Manchester Integrated Board. Facilitating move on from funded placements where appropriate.
