Job Summary
We are seeking a dedicated and compassionate Care Manager to oversee the delivery of high-quality care to our service users who live in their own homes in the community.
Pay to be discussed dependent on experience.
The ideal candidate will possess strong leadership skills and a thorough understanding of support plans and risk assessments, ensuring that our service users receive personalised support tailored to their individual needs. This role requires a proactive approach to managing staff, fostering a positive environment for both our service users and team members, and maintaining compliance with regulatory standards.
Responsibilities
* Develop, implement, and monitor individual support plans for service users, ensuring they meet their specific needs.
* Supervise and lead a team of care staff, providing guidance, support, and training as necessary.
* Conduct regular assessments of service users' health and wellbeing to adjust care plans accordingly.
* Collaborate with healthcare professionals, families, and other stakeholders to ensure comprehensive care delivery.
* Maintain accurate records of service users' care and staff performance in accordance with regulatory requirements.
* Promote a safe and supportive environment.
* Address any concerns or complaints from service users or their families promptly and effectively.
* Deal with any safeguarding concerns.
* Be available to support and sometimes attend visits to ensure that all service users receive their visits.
* Assess and implement new packages.
* Carry out regular audits of the service.
* Ensure that all records are completed and any actions are addressed.
* Manage recruitment and retention.
* Ensure that you are fully up to date with all CQC regulations and requirements.
Requirements
* Proven experience in domiciliary care is essential.
* Strong knowledge of support plans and the ability to manage multiple cases effectively.
* Excellent leadership skills with the ability to motivate and supervise a diverse team.
* Relevant qualifications in healthcare management are preferred.
* Exceptional communication skills, both verbal and written, to liaise with service users, families, and staff.
* A compassionate approach towards elderly care with a commitment to enhancing the quality of life for our service users.
* Ability to work flexibly to meet the needs of the service.
