Main Duties:
1. Enhance Health and Wellbeing: Promote the health and wellbeing of individuals with complex diabetes, helping them achieve optimal glycaemic control through informed self-management.
2. Empower Self-Management: Support and empower individuals to manage their diabetes effectively.
3. Educational Support: Offer education and guidance to individuals with diabetes and their carers.
4. Foster Collaborative Working: Encourage and facilitate regular collaboration among all healthcare professionals involved in diabetes care.
5. Facilitate Communication: Serve as a liaison between GP and consultant-led specialist teams.
Requirements of the Diabetes Specialist Nurse:
1. Must have a degree/equivalent within Nursing (RGN qualified) and an active NMC Registration (Adult).
2. Experience of working with patients with complex diabetes.
