Locum Consultant Physician in Care of the Elderly

The post is a new locum post, and the successful applicant will join a Community team with aim of overseeing Frailty in Primary and Community Services and supporting with transformational change across the care group.
The post will be subject to the terms and conditions of service governing the appointment of Medical and Dental Staff in Wales, and to the Health Board's terms and conditions as appropriate.
The post holder will be part of a dynamic, forward looking and innovative Primary and Community team and can be based at any community site in the Health Board. It will be part of the wider primary and community team which includes support for the urgent community pathway for frailty, which will support effective community support for ED assessment and turnaround, and development of urgent and emergency frailty pathway of support in the community.

Main duties of the job
* To be involved in rehabilitation and discharge planning for frail elderly patients.
* To provide holistic care for those with frailty.
* Contribute to the support and management of appropriate and End of Life care.
* To provide advice and support to medical teams dealing with patients with complicated Rehabilitation/Discharge Planning issues.
* Lead on delivery of high-quality care in our community bed facilities to serve increasing and more enhanced "step up" models of care, as well as rehabilitation.
* Specialist community MDT working (virtual ward models for planning care of complex patients) in partnership with Social Care, Mental Health, Primary Care, Specialist Community Nursing, Therapy, End of Life and emergency services.
* Lead on clinically safe alternatives to acute admission in the community.
* The post holder will augment the ongoing education and training of medical and allied health professionals in Frailty and the management of acute frailty syndrome presentations.

Person Specification
Teaching/ Research Experience
* Experience of supervising Foundation Trainees and SHOs/ST1s and ST2s.
* Ability to teach clinical skills.
* Ability to apply research outcomes to clinical problems.
* Publications in peer reviewed journal Involvement with Deanery/RCP.
Education/ Qualifications and training
* GMC Registration with a licence to practice.
* Possession of MRCP (UK) or equivalent qualification.
* Sub-specialty training which complements Care of the Elderly Medicine.
Clinical Experience
* Ability to offer expert clinical opinion on a range of medical problems in Medicine for the Elderly.
* Ability to take full and independent responsibility for clinical care of patients.
* Experience of working at a higher level e.g. Speciality Doctor / Locum Consultant at present.
* Experience in Frailty management & Acute Geriatrics.
* Having experience of setting up a service.
Management and administrative experience
* Ability to run an efficient and effective acute service.
* Ability to organise and manage care of patients within both acute unit and outpatient areas.
* Ability to manage and lead a multidisciplinary team including junior doctors and therapists.
* Experience of audit.
* Demonstrate drive, achievement and innovation to establish and develop new and innovative service.
Other attributes
* Ability to work in a multi-professional team.
* Good interpersonal skills.
* Caring attitude to patients.
* Ability to communicate effectively - with patients, relatives, GPs, nurses and others.
* Commitment to Continuing Medical Education.
