This is a fantastic opportunity for a physiotherapist to join the community reablement team who offer multi-disciplinary therapy in the community in Rhondda, Cynon and Taf areas of Cwm Taf Morgannwg.
This service will provide triage, assessment, and therapy intervention when needed at local communities, mainly in the service users' homes. The service sits within the local authority service and has excellent links with local authority providers and the third sector.
The team will work with an individual and their family/carers to deliver goal-oriented therapy programmes in the home environment.
Main duties of the job
To provide a high standard of physiotherapy service within the community setting working across the Cwm Taf Morgannwg University Health Board (CTMUHB) localities, health board sites, and various community settings.
To autonomously work as well as alongside members of the multidisciplinary team ensuring best current, evidence-based clinical management of patients referred within the defined specialist clinical area.
The post holder will work as an autonomous practitioner and will have relevant post-registration experience for the role and will undertake specialist clinical duties as required within a defined specialist clinical area.

Person Specification
Qualifications/ Knowledge
* BSc Physiotherapy
* Health Professions Council registration
* Knowledge of clinical guidelines and standards for community-based rehabilitation
* Sound knowledge of Standards of Professional Practice
* Knowledge of a range of policies/procedures.
* Completion of validated Clinical Educator's course
* Evidence of attendance on appropriate courses to enhance specialist knowledge/skills
Experience
* Full experience within the core Band 5 rotations
* Experience of community working
* NHS or social services experience
* Experience of working in a multi-disciplinary setting
* Experience in individual and group work
Aptitude and Abilities
* Ability to work independently and as part of a multi-disciplinary team
* Ability to keep accurate and legible patient notes
* Ability to organise and prioritise
* Good time management
